Create or update a practice PUT

Profile, locale, tooth numbering, currency and sites. Creates the hidden clinic and its system owner on first call; sites missing from the list are deactivated. A closed practice is never revived.

Ask a lab to connect POST

Sends the lab a partnership request from the practice. A live relationship with that lab is answered as it stands, so replays are safe.
