Profile, locale, tooth numbering, currency and sites. Creates the hidden clinic and its system owner on first call; sites missing from the list are deactivated. A closed practice is never revived.

Create or update a patient PUT

Keyed by the Smileline patient id. A patient Smileline has asked to erase is refused for good.

Remove a user DELETE

Removes the user's membership and keeps a tombstone at this authority version, so an older grant arriving later is refused.
